I understand that there are certain risks involved with participating in any recreational activity. I expressly understand that the Happy Smackah planning committee, nor any of its officers, agents, volunteers, or assistants shall be held responsible or made subject to any claims, including any claim for negligence, seeking to assess damages or liability for or arising from personal injury or property damage to myself or other person in whose behalf this form is submitted as a result of actual or proposed participation in the event. I am over 18 years of age OR the parent/legal guardian of a minor under 18 years of age OR the legal guardian of an incapacitated and/or mentally challenged person with legal standing to accept the terms of this liability waiver agreement. I, on behalf of myself and/or my child, hereby agree to HOLD THE HAPPY SMACKAH VOLUNTEERS, OFFICERS, AGENTS, AND ASSISTANTS HARMLESS ON ACCOUNT OF ANY SUCH CLAIM.
